Dissection is done with blunt elements, so as to avoid cutting any noble structure you may find in the cheek.
Does the removal of bichat balls cause facial flaccidity?
No. What it causes is a sagging at the mid-cheek level. There is not such a large volume of fat extraction as to cause sagging either in an older person or in the long term.
